Prescription Refills
If you need a refill on one of your pets' medications please fill out the form below and we will get back to you as soon as we can. If we do not have the medication in stock please allow 1 to 2 days for a refill and we will let you know via your preferred method of communication as soon as we have the medication.
**Please note that we require a physical exam to be performed on your pet by the doctor prior to any prescription refills at LEAST once a year and sometimes more often depending on the situation or medication. Only CPOAC clients may use this form to submit medication refill requests.
